UNABLE TO ATTACH DOCUMENT.
HOW DOES ONE ATTACH A DOCUMENT WHEN IN THE 'ENTER ISSUE' SCREEN?

Using version 2.01. Had same problem with version 2.00

Table of Contents when inserted corrupts document.
Extra pages are added.
Suspect it has something to do with pictures that were placed in document.

Use the attached file and insert a Table of Contents on page 2 of the document.
Notice the page numbers and how the document has changed.
It might be of help to use the Edit menu to observe what Undo message is 
displayed. 
Using the Delete button at the last line of where the problem occurs corrects 
the document.
But the page numbers in the Table of Contents have to be manually corrected.

MRU->OD: go to page 2 and update the TOC. Then a few pages loweryou can see that
there is a lot of spacing above one of the graphics. When moving the grahic
slightly, the position will be corrected.
