I've some chapter-paragraphs (level 5) in table-cells. 
When I create a TOC the order is correct, same sequence as in the document.
When I open the Navigator, these chapters appear at the top of the heading-list,
before chapter 1.
The same after export as PDF: this chapters do not appear integrated in the
content-order but at the top of the bookmark-list, before chapter 1.

Sorry. I've to correct myself: the problem does not exist because of the
table-cell but the reason is a frame around the chapter-paragraph.
In my file the frame is part of a cell so I first thought, the problem is the table.
But I made a little example to demonstrate.
When you open the navigator you see at the top to headings that should be beyond
chapter 3 and chapter 4.
The same occurs when you export to pdf: The bookmark-list is not in the order of
the TOC
